Valsartan + Sacubitril

Tablet

This fixed-dose combination (ARNI) is used in the management of heart failure with reduced ejection fraction (HFrEF). Valsartan, an angiotensin receptor blocker (ARB), reduces vasoconstriction and aldosterone effects, while Sacubitril, a neprilysin inhibitor, enhances natriuretic peptides, leading to vasodilation, natriuresis, and reduced cardiac remodeling.

Mechanism

Valsartan: Blocks angiotensin II AT1 receptors → vasodilation, reduced aldosterone secretion, decreased blood pressure.
Sacubitril: Inhibits neprilysin → increases levels of natriuretic peptides → vasodilation, natriuresis, diuresis, and reduced cardiac remodeling.

Pharmacokinetics

  • Absorption: Both well absorbed orally; peak plasma levels in 1–2 hrs.
  • Distribution: Valsartan highly protein bound (~95%); Sacubitril moderately bound.
  • Metabolism: Sacubitril converted to active metabolite LBQ657; Valsartan minimally metabolized.
  • Excretion: Valsartan via feces; Sacubitril metabolite via urine.
  • Half-life: Valsartan ~9 hrs; LBQ657 ~11 hrs.

Side Effects

  • Hypotension
  • Hyperkalemia
  • Cough
  • Dizziness
  • Rare: Angioedema

Contraindication

  • History of angioedema with ACE inhibitors or ARBs
  • Pregnancy
  • Severe hepatic impairment

Interaction

  • Potassium-sparing diuretics ↑ risk of hyperkalemia
  • NSAIDs may reduce antihypertensive effect
  • Do not combine with ACE inhibitors (↑ risk of angioedema)

Warning and Precautions

  • Monitor blood pressure, renal function, and potassium levels
  • Discontinue ACE inhibitors at least 36 hrs before starting therapy
  • Use cautiously in renal impairment
