Valsartan + Amlodipine

Tablet

Amlodipine + Valsartan is a fixed-dose combination of a calcium channel blocker (CCB) and an angiotensin II receptor blocker (ARB). It provides dual antihypertensive action by reducing vascular resistance and blocking vasoconstriction, offering effective blood pressure control in patients not adequately managed with monotherapy.

Mechanism

Amlodipine inhibits calcium influx into vascular smooth muscle, reducing peripheral resistance and lowering blood pressure. Valsartan selectively blocks angiotensin II type 1 (AT1) receptors, preventing vasoconstriction and aldosterone release. Together, they provide complementary antihypertensive effects.

Pharmacokinetics

  • Amlodipine: Bioavailability ~60–65%, half-life ~30–50 hours
  • Valsartan: Bioavailability ~25%, half-life ~6 hours
  • Excretion: Amlodipine via urine; Valsartan via bile/feces

Side Effects

  • Dizziness
  • Peripheral edema
  • Hypotension
  • Fatigue
  • Hyperkalemia (rare)
  • Flushing or headache

Contraindication

  • Pregnancy (fetal toxicity risk due to ARB)
  • Hypersensitivity to Amlodipine, Valsartan, or excipients
  • Severe hepatic impairment
  • Concomitant use with aliskiren in diabetic patients

Interaction

  • ACE inhibitors or aliskiren: ↑ risk of renal impairment & hyperkalemia
  • Potassium supplements/diuretics: ↑ serum potassium
  • NSAIDs: ↓ antihypertensive effect, possible renal impairment
  • CYP3A4 inhibitors: ↑ plasma levels of Amlodipine

Warning and Precautions

  • Monitor renal function and electrolytes
  • Risk of hypotension in volume-depleted patients
  • Peripheral edema risk due to Amlodipine
  • Use caution in hepatic impairment
