Mechanism

Valacyclovir is rapidly converted to acyclovir after oral administration. Acyclovir inhibits viral DNA polymerase, preventing replication of herpes simplex virus (HSV) and varicella-zoster virus (VZV).

Pharmacokinetics

  • Absorption: Well absorbed orally.
  • Distribution: Widely distributed; reaches lungs and kidneys.
  • Metabolism: Minimal hepatic metabolism.
  • Elimination: Primarily renal excretion.
  • Half-life: ~3–4 hours.

Side Effects

  • Headache
  • Nausea and abdominal pain
  • Confusion or hallucinations (rare, especially in elderly)
  • Kidney dysfunction (rare)

Contraindication

  • Hypersensitivity to valacyclovir or acyclovir
  • History of severe allergic skin reactions

Interaction

  • Nephrotoxic drugs (e.g., aminoglycosides, cyclosporine, tacrolimus) increase risk of renal toxicity
  • Other antivirals (e.g., foscarnet, tenofovir) may enhance renal side effects
  • Concurrent use with cimetidine or probenecid may increase acyclovir levels

Warning and Precautions

  • Start therapy as early as possible after symptom onset
  • Maintain adequate hydration to protect kidney function
  • Use with caution in elderly patients
  • Does not prevent transmission — safe sex practices are essential
