Mechanism

Tramadol: Centrally acting synthetic opioid → binds to μ-opioid receptors and inhibits reuptake of norepinephrine and serotonin → analgesic effect.
Paracetamol: Inhibits prostaglandin synthesis in CNS → antipyretic and analgesic effect.

Pharmacokinetics

  • Absorption: Both rapidly absorbed orally; peak plasma levels in 1–2 hrs.
  • Distribution: Tramadol widely distributed, ~20% protein bound; Paracetamol widely distributed, low protein binding.
  • Metabolism: Tramadol metabolized hepatically via CYP2D6 & CYP3A4; Paracetamol via glucuronidation & sulfation.
  • Excretion: Both primarily excreted via urine.
  • Half-life: Tramadol ~6 hrs; Paracetamol ~2–3 hrs.

Side Effects

  • Nausea, vomiting
  • Dizziness, drowsiness
  • Constipation
  • Risk of dependence (Tramadol)
  • Hepatotoxicity with overdose (Paracetamol)

Contraindication

  • Hypersensitivity to Tramadol or Paracetamol
  • Severe hepatic impairment
  • History of opioid dependence
  • Acute intoxication with alcohol or CNS depressants

Interaction

  • Alcohol increases hepatotoxicity risk (Paracetamol)
  • Tramadol may interact with SSRIs, MAO inhibitors (↑ serotonin syndrome risk)
  • Other CNS depressants increase sedation

Warning and Precautions

  • Do not exceed recommended daily dose
  • Monitor liver function during prolonged use
  • Use cautiously in patients with seizure disorders
  • Risk of dependence with long-term use

Dosage

As directed by the Physician.
