Mechanism

Tinidazole enters protozoal and bacterial cells, where its nitro group is reduced by intracellular transport proteins. The resulting reactive intermediates damage DNA and other critical biomolecules, leading to cell death.

Pharmacokinetics

  • Absorption: Rapid oral absorption, bioavailability ≈ 100%
  • Distribution: Widely distributed, crosses placenta and enters breast milk
  • Metabolism: Hepatic metabolism via oxidation and conjugation
  • Elimination: Renal and fecal excretion
  • Half-life: ≈ 12–14 hours

Side Effects

  • Nausea and vomiting
  • Metallic taste
  • Abdominal discomfort
  • Dizziness
  • Rare: peripheral neuropathy

Contraindication

  • First trimester of pregnancy
  • Hypersensitivity to nitroimidazoles
  • Neurological disorders (use cautiously)

Interaction

  • Alcohol — disulfiram-like reaction (avoid during and 72 hours after therapy)
  • Warfarin — enhances anticoagulant effect
  • CYP3A4 inducers (rifampicin, phenytoin) — reduce plasma levels
  • CYP3A4 inhibitors (ketoconazole) — increase plasma levels

Warning and Precautions

  • Avoid alcohol during and after therapy
  • Monitor liver function in prolonged use
  • Use cautiously in patients with CNS disease
