Terbinafine

Tablet

Terbinafine is a synthetic allylamine antifungal agent. It works by inhibiting the fungal enzyme squalene epoxidase, leading to accumulation of squalene and disruption of ergosterol synthesis. This results in fungal cell death. Terbinafine is highly effective in treating nail, skin, and scalp fungal infections.

Mechanism

Terbinafine inhibits squalene epoxidase, blocking ergosterol synthesis in fungal cell membranes. This leads to accumulation of toxic squalene and fungal cell death. It is fungicidal against dermatophytes and fungistatic against yeasts.

Pharmacokinetics

  • Absorption: Well absorbed orally
  • Distribution: Widely distributed in skin, nails, and adipose tissue
  • Metabolism: Hepatic metabolism via CYP enzymes
  • Elimination: Renal and fecal excretion
  • Half-life: 24–36 hours

Side Effects

  • Gastrointestinal upset (nausea, diarrhea)
  • Headache
  • Skin rash
  • Liver enzyme elevation (rare)

Contraindication

  • Hypersensitivity to Terbinafine
  • Severe hepatic impairment

Interaction

  • CYP2D6 substrates (e.g., tricyclic antidepressants, beta-blockers)
  • Rifampicin (reduces Terbinafine levels)
  • Cimetidine (increases Terbinafine levels)

Warning and Precautions

  • Monitor liver function during prolonged therapy
  • Use cautiously in patients with renal impairment
  • Discontinue if rash or allergic reaction occurs
