Mechanism

Spironolactone: Aldosterone antagonist, reduces sodium reabsorption and conserves potassium.
Frusemide: Inhibits sodium-potassium-chloride transporter in the loop of Henle, producing potent diuresis.

Pharmacokinetics

  • Spironolactone: Absorbed orally; metabolized in liver to active metabolites; half-life ~1.5 hours (metabolites longer); excreted via urine and bile.
  • Frusemide: Rapid absorption; peak plasma concentration in 1–2 hours; half-life ~1–2 hours; excreted unchanged in urine.

Side Effects

  • Hyperkalemia (due to Spironolactone)
  • Hypokalemia, dehydration (due to Frusemide)
  • Dizziness, hypotension
  • Gynecomastia (Spironolactone)

Contraindication

  • Severe renal impairment
  • Anuria
  • Hyperkalemia
  • Hypersensitivity to either drug

Interaction

  • ACE inhibitors/ARBs (increase risk of hyperkalemia)
  • NSAIDs (reduce diuretic efficacy)
  • Digoxin (risk of toxicity with electrolyte imbalance)

Warning and Precautions

  • Use under medical supervision only
  • Monitor electrolytes regularly
  • Caution in elderly patients
  • Avoid potassium supplements unless prescribed
