Mechanism

Sirolimus binds to FKBP-12, forming a complex that inhibits mTOR (mammalian target of rapamycin). This blocks T-cell activation and proliferation, reducing immune-mediated rejection and exerting antiproliferative effects.

Pharmacokinetics

  • Absorption: Oral bioavailability ~15%; peak plasma concentration in 1–2 hours.
  • Protein Binding: ~92% bound to plasma proteins.
  • Metabolism: Extensively metabolized in the liver via CYP3A4.
  • Excretion: Primarily via feces.
  • Half-life: ~60 hours.

Side Effects

  • Hyperlipidemia
  • Thrombocytopenia
  • Delayed wound healing
  • Increased risk of infections

Contraindication

  • Hypersensitivity to Sirolimus
  • Severe hepatic impairment (caution)
  • Pregnancy and breastfeeding

Interaction

  • CYP3A4 inhibitors (e.g., ketoconazole, erythromycin) increase Sirolimus levels
  • CYP3A4 inducers (e.g., rifampin) decrease Sirolimus levels
  • Other immunosuppressants (additive effect)

Warning and Precautions

  • Use under strict medical supervision
  • Monitor blood levels regularly
  • Monitor lipid profile and blood counts
  • Avoid live vaccines during therapy
