Simvastatin

Tablet

Simvastatin is a lipid-lowering agent belonging to the statin class. It reduces elevated total cholesterol, LDL cholesterol, and triglycerides while increasing HDL cholesterol. By lowering cholesterol levels, Simvastatin helps prevent cardiovascular events such as heart attacks and strokes, especially in patients with hyperlipidemia or high cardiovascular risk.

Mechanism

Simvastatin is a prodrug that is hydrolyzed to its active β-hydroxyacid form. It selectively inhibits HMG-CoA reductase, the enzyme responsible for converting HMG-CoA to mevalonate, a precursor of cholesterol. This inhibition decreases hepatic cholesterol synthesis, upregulates LDL receptors, and enhances clearance of LDL cholesterol from the bloodstream.

Pharmacokinetics

  • Bioavailability: ~5%
  • Peak plasma concentration: 1.3–2.4 hours
  • Half-life: ~2 hours
  • Metabolism: Hepatic via CYP3A4
  • Excretion: Primarily via feces

Side Effects

  • Headache, nausea, abdominal pain
  • Myalgia, muscle cramps
  • Elevated liver enzymes
  • Rare: rhabdomyolysis, severe myopathy

Contraindication

  • Active liver disease or unexplained persistent elevations of liver enzymes
  • Pregnancy and breastfeeding
  • Hypersensitivity to Simvastatin or excipients
  • Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)

Interaction

  • Strong CYP3A4 inhibitors: ↑ risk of myopathy/rhabdomyolysis
  • Fibrates and niacin: ↑ risk of muscle toxicity
  • Warfarin: ↑ anticoagulant effect
  • Grapefruit juice: ↑ plasma concentration of Simvastatin

Warning and Precautions

  • Monitor liver function before and during therapy
  • Risk of myopathy and rhabdomyolysis, especially at higher doses
  • Discontinue if unexplained muscle pain or weakness occurs
  • Use caution in patients with renal impairment
