Mechanism

Rizatriptan binds selectively to serotonin 5-HT1B and 5-HT1D receptors, causing vasoconstriction of dilated cranial blood vessels and inhibiting release of pro-inflammatory neuropeptides. This dual action alleviates migraine pain and associated symptoms.

Pharmacokinetics

  • Bioavailability: ~40–45%
  • Peak plasma concentration: 1–1.5 hours
  • Half-life: ~2–3 hours
  • Metabolism: Hepatic via monoamine oxidase-A (MAO-A)
  • Excretion: Primarily via urine

Side Effects

  • Dizziness
  • Fatigue
  • Somnolence
  • Chest discomfort
  • Rare: myocardial ischemia, arrhythmia

Contraindication

  • Hypersensitivity to Rizatriptan
  • History of ischemic heart disease, stroke, or uncontrolled hypertension
  • Concomitant use with MAO inhibitors
  • Use within 24 hours of another triptan or ergotamine derivative

Interaction

  • MAO-A inhibitors: ↑ plasma levels of Rizatriptan
  • Propranolol: ↑ Rizatriptan concentration, dose adjustment required
  • Other triptans/ergot derivatives: ↑ risk of vasospasm

Warning and Precautions

  • Assess cardiovascular status before prescribing
  • Not recommended in patients with uncontrolled hypertension
  • Use caution in hepatic impairment
  • Discontinue if chest pain or ischemic symptoms occur

Dosage

As directed by the Physician.
