Mechanism

Risperidone blocks dopamine D2 and serotonin 5-HT2 receptors, reducing psychotic symptoms. Trihexyphenidyl blocks central cholinergic receptors, preventing extrapyramidal side effects.

Pharmacokinetics

  • Absorption: Risperidone well absorbed orally; Trihexyphenidyl rapidly absorbed.
  • Metabolism: Risperidone metabolized hepatically via CYP2D6; Trihexyphenidyl metabolized in liver.
  • Excretion: Risperidone via urine; Trihexyphenidyl via urine and feces.
  • Half-life: Risperidone ~20 hours; Trihexyphenidyl ~3–4 hours.

Side Effects

  • Drowsiness
  • Weight gain
  • Dry mouth
  • Blurred vision
  • Constipation
  • Rare: Tardive dyskinesia, Neuroleptic Malignant Syndrome

Contraindication

  • Hypersensitivity to risperidone or trihexyphenidyl
  • Angle-closure glaucoma
  • Severe hepatic impairment

Interaction

  • Other CNS depressants (additive sedation)
  • Antihypertensives (enhanced hypotension)
  • Anticholinergics (additive side effects)

Warning and Precautions

  • Monitor for metabolic changes (weight, glucose, lipids)
  • Use cautiously in elderly patients
  • Discontinue if severe EPS or tardive dyskinesia occurs
  • Not recommended during pregnancy unless essential
