Strength
  • Repaglinide 1 mg/li>
  • Metformin 500 mg
Packing Size
  • 10 Tablets

Mechanism

Repaglinide stimulates insulin release from pancreatic beta cells by closing ATP-dependent potassium channels. Metformin decreases hepatic glucose production, increases peripheral glucose uptake, and reduces intestinal glucose absorption.

Pharmacokinetics

  • Absorption: Rapid oral absorption.
  • Metabolism: Repaglinide via CYP3A4; Metformin not metabolized.
  • Excretion: Repaglinide via bile; Metformin via urine.
  • Half-life: Repaglinide ~1 hour; Metformin ~6 hours.

Side Effects

  • Hypoglycemia
  • Weight gain
  • Gastrointestinal upset (nausea, diarrhea)
  • Headache

Contraindication

  • Type 1 Diabetes Mellitus
  • Diabetic ketoacidosis
  • Severe renal impairment (eGFR <30 mL/min/1.73 m²)
  • Hepatic impairment

Interaction

  • Gemfibrozil (increases repaglinide levels → hypoglycemia)
  • Alcohol (risk of lactic acidosis with metformin)
  • Beta-blockers (mask hypoglycemia symptoms)
  • CYP3A4 inhibitors (clarithromycin, ketoconazole → ↑ repaglinide)

Warning and Precautions

  • Use under medical supervision only
  • Monitor renal function before and during therapy
  • Discontinue if lactic acidosis is suspected
  • Advise patients to avoid excessive alcohol intake

Dosage

As directed by the Physician.
