Mechanism

Ramipril inhibits angiotensin-converting enzyme, reducing angiotensin II formation and relaxing blood vessels. Losartan blocks angiotensin II receptors, preventing vasoconstriction and aldosterone release. Together, they lower blood pressure and protect the heart and kidneys.

Pharmacokinetics

  • Absorption: Rapid oral absorption.
  • Distribution: Wide tissue penetration.
  • Metabolism: Ramipril metabolized in liver to ramiprilat; Losartan metabolized to active metabolite (EXP3174).
  • Elimination: Renal and biliary excretion.
  • Half-life: Ramiprilat ~13–17 hrs; Losartan metabolite ~6–9 hrs.

Side Effects

  • Dizziness, headache
  • Cough (ACE inhibitor effect)
  • Fatigue
  • Electrolyte imbalance (hyperkalemia)
  • Rare: angioedema, kidney dysfunction

Contraindication

  • Pregnancy and lactation
  • Severe renal impairment
  • History of angioedema
  • Concomitant use with aliskiren in diabetic nephropathy

Interaction

  • NSAIDs (reduce effect, increase kidney risk)
  • Potassium supplements/diuretics (risk of hyperkalemia)
  • Lithium (toxicity risk)
  • Alcohol (enhances hypotension)

Warning and Precautions

  • Monitor blood pressure, kidney function, and electrolytes regularly
  • Avoid abrupt discontinuation
  • Use cautiously in elderly patients
