Mechanism

Propranolol competitively blocks beta-1 and beta-2 adrenergic receptors. This reduces sympathetic stimulation of the heart, lowering heart rate, myocardial contractility, and blood pressure. It also reduces renin release from the kidneys and stabilizes neuronal activity in migraine prophylaxis.

Pharmacokinetics

  • Absorption: Rapid oral absorption; bioavailability ~25% due to first-pass metabolism.
  • Distribution: Highly lipophilic; crosses blood-brain barrier.
  • Metabolism: Extensive hepatic metabolism via CYP2D6.
  • Elimination: Excreted in urine as metabolites.
  • Half-life: ~3–6 hours.

Side Effects

  • Bradycardia
  • Hypotension
  • Fatigue
  • Dizziness
  • Cold extremities
  • Sleep disturbances, vivid dreams
  • Rare: bronchospasm, depression

Contraindication

  • Hypersensitivity to Propranolol
  • Bronchial asthma or COPD (risk of bronchospasm)
  • Severe bradycardia
  • Second or third-degree heart block
  • Cardiogenic shock

Interaction

  • Calcium channel blockers: Additive bradycardia and hypotension.
  • Antiarrhythmics: Increased risk of conduction abnormalities.
  • NSAIDs: May reduce antihypertensive effect.
  • Insulin/antidiabetics: May mask hypoglycemia symptoms.

Warning & Precautions

  • Do not discontinue abruptly — risk of rebound hypertension/angina.
  • Monitor heart rate and blood pressure regularly.
  • Use cautiously in diabetics and patients with depression.
  • Adjust dose in hepatic impairment.
