Mechanism

Pimozide selectively blocks dopamine D2 receptors in the mesolimbic and nigrostriatal pathways. This reduces dopaminergic hyperactivity, controlling psychotic symptoms and suppressing motor/vocal tics in Tourette’s disorder.

Pharmacokinetics

  • Absorption: Well absorbed orally
  • Distribution: Highly protein bound (~95%)
  • Metabolism: Hepatic via CYP3A4
  • Half-life: ~55 hours (long duration of action)
  • Excretion: Metabolites excreted in urine and feces

Side Effects

  • Drowsiness
  • Extrapyramidal symptoms (tremor, rigidity)
  • Dry mouth
  • Constipation
  • QT prolongation (serious but rare)

Contraindication

  • Hypersensitivity to Pimozide
  • History of cardiac arrhythmias or QT prolongation
  • Concomitant use with CYP3A4 inhibitors (e.g., ketoconazole, macrolides)

Interaction

  • CYP3A4 inhibitors: ↑ Pimozide levels, ↑ risk of arrhythmias
  • Other antipsychotics: additive extrapyramidal effects
  • QT-prolonging drugs: ↑ risk of torsades de pointes

Warning and Precautions

  • Monitor ECG regularly (risk of QT prolongation)
  • Use caution in patients with electrolyte imbalance
  • Gradual dose titration recommended
  • Not recommended in children under 12 years

Dosage

As directed by the Physician.
