Mechanism

Paracetamol: Inhibits prostaglandin synthesis in the CNS, reducing pain and fever.
Caffeine: Stimulates CNS, increases alertness, and potentiates analgesic action.
Chlorpheniramine: Blocks H1 histamine receptors, reducing allergy symptoms like sneezing and runny nose.

Pharmacokinetics

  • Absorption: Rapidly absorbed from GI tract.
  • Distribution: Paracetamol widely distributed; Caffeine crosses BBB; Chlorpheniramine distributes to tissues.
  • Metabolism: Paracetamol metabolized in liver; Caffeine metabolized by CYP1A2; Chlorpheniramine metabolized hepatically.
  • Elimination: Paracetamol via urine; Caffeine via urine; Chlorpheniramine via renal excretion.
  • Half-life: Paracetamol ~2–3 hrs; Caffeine ~5 hrs; Chlorpheniramine ~20 hrs.

Side Effects

  • Drowsiness (Chlorpheniramine)
  • Insomnia or jitteriness (Caffeine)
  • Nausea, vomiting
  • Rare: Liver toxicity (Paracetamol overdose)
  • Dry mouth, blurred vision

Contraindication

  • Hypersensitivity to any component
  • Severe hepatic impairment
  • Cardiac arrhythmias (avoid caffeine)
  • Glaucoma (avoid chlorpheniramine)
  • Pregnancy and lactation (use with caution)

Interaction

  • Alcohol (increases hepatotoxicity risk with Paracetamol)
  • MAO inhibitors (enhanced anticholinergic effect with Chlorpheniramine)
  • CNS stimulants (additive effect with Caffeine)
  • Warfarin (Paracetamol may increase INR with prolonged use)

Warning and Precautions

  • Do not exceed recommended Paracetamol dose (risk of liver damage)
  • Use cautiously in patients with hypertension or arrhythmias
  • May cause drowsiness — avoid driving or operating machinery
  • Monitor liver function in long-term use
