Mechanism

Omeprazole: Inhibits H+/K+ ATPase pump in gastric parietal cells → reduces gastric acid secretion.
Ondansetron: Blocks serotonin 5-HT3 receptors in the brain and GI tract → prevents nausea and vomiting reflex.

Pharmacokinetics

  • Omeprazole Absorption: Rapidly absorbed; peak plasma levels in 0.5–3.5 hrs.
  • Ondansetron Absorption: Peak plasma levels in 1–2 hrs.
  • Distribution: Omeprazole ~95% protein bound; Ondansetron ~70% bound.
  • Metabolism: Omeprazole via CYP2C19 and CYP3A4; Ondansetron via CYP3A4, CYP2D6, CYP1A2.
  • Excretion: Omeprazole via urine and feces; Ondansetron via urine.
  • Half-life: Omeprazole ~1 hr; Ondansetron ~3–6 hrs.

Side Effects

  • Headache
  • Constipation or diarrhea
  • Dizziness
  • Abdominal pain
  • Rare: Allergic reactions, QT prolongation (Ondansetron)

Contraindication

  • Hypersensitivity to Omeprazole or Ondansetron
  • Severe hepatic impairment
  • Concurrent use with apomorphine (Ondansetron contraindicated)

Interaction

  • Omeprazole may reduce absorption of drugs requiring acidic pH (e.g., ketoconazole)
  • Ondansetron may interact with serotonergic drugs (↑ risk of serotonin syndrome)
  • Both may interact with CYP450 metabolized drugs

Warning and Precautions

  • Severe hepatic impairment
  • Monitor ECG in patients at risk of QT prolongation
  • Use cautiously in hepatic impairment
  • Long-term omeprazole use may cause vitamin B12 deficiency
