Mechanism

Ofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, preventing DNA replication and cell division. Ornidazole disrupts DNA synthesis in anaerobic bacteria and protozoa, leading to cell death. Together, they provide broad-spectrum coverage against bacterial and protozoal pathogens.

Pharmacokinetics

  • Absorption: Rapid oral absorption; bioavailability ≈ 90%
  • Distribution: Widely distributed in body tissues and fluids
  • Metabolism: Ofloxacin — hepatic metabolism; Ornidazole — extensive hepatic metabolism
  • Elimination: Primarily renal excretion
  • Half-life: Ofloxacin ≈ 5–7 hours; Ornidazole ≈ 12–15 hours

Side Effects

  • Nausea
  • Diarrhea
  • Headache
  • Dizziness
  • Metallic taste
  • Rare: tendon rupture, peripheral neuropathy

Contraindication

  • Hypersensitivity to fluoroquinolones or nitroimidazoles
  • History of tendon disorders related to quinolone use
  • Pregnancy and lactation (use only if clearly needed)
  • Epilepsy or CNS disorders

Interaction

  • Antacids — reduce absorption of Ofloxacin
  • Warfarin — increased anticoagulant effect with Ornidazole
  • Alcohol — avoid due to disulfiram-like reaction with Ornidazole
  • NSAIDs — may increase risk of seizures with fluoroquinolones

Warning and Precautions

  • Use cautiously in patients with hepatic or renal impairment
  • Avoid alcohol during and 48 hours after therapy
  • Monitor for signs of tendon inflammation or rupture
  • Discontinue if neurological symptoms occur
  • Prolonged use may lead to superinfection
