Mechanism

Nimesulide: Selective COX-2 inhibitor → reduces prostaglandin synthesis → pain & inflammation relief.
Serratiopeptidase: Proteolytic enzyme → breaks down inflammatory mediators → reduces swelling & promotes healing.

Pharmacokinetics

  • Absorption: Nimesulide is rapidly absorbed orally with peak plasma levels in 1.5–3 hrs; Serratiopeptidase is absorbed from the intestine and reaches systemic circulation bound to alpha-2 macroglobulin.
  • Distribution: Nimesulide is ~99% plasma protein bound; Serratiopeptidase distributes to inflamed tissues.
  • Metabolism: Nimesulide undergoes hepatic metabolism via CYP enzymes; Serratiopeptidase is metabolized as a protein enzyme.
  • Excretion: Nimesulide is primarily excreted via urine and bile; Serratiopeptidase is eliminated through renal pathways.
  • Half-life: Nimesulide ~2–5 hrs; Serratiopeptidase ~1 hr (enzyme activity dependent).

Side Effects

  • Gastrointestinal upset
  • Hepatotoxicity risk (Nimesulide)
  • Allergic reactions (rare)
  • Nausea or dizziness

Contraindication

  • Severe hepatic impairment
  • Hypersensitivity to NSAIDs or enzymes
  • Pregnancy & lactation (use cautiously)

Interaction

  • Alcohol increases hepatotoxicity risk with Nimesulide
  • May interact with anticoagulants (↑ bleeding risk)
  • Other NSAIDs increase GI side effect risk

Warning and Precautions

  • Do not exceed recommended daily dose
  • Monitor liver function during prolonged use
  • Use cautiously in patients with bleeding disorders
