Mechanism

Nimesulide: Inhibits COX-2 enzyme, reducing prostaglandin synthesis and inflammation.
Paracetamol: Acts centrally to reduce fever and pain by inhibiting prostaglandin synthesis in the CNS.
Cetirizine: Antihistamine that blocks H1 receptors, reducing allergy symptoms.
Phenylpropanolamine: Sympathomimetic agent that reduces nasal congestion by vasoconstriction.
Caffeine: CNS stimulant that reduces drowsiness and enhances alertness.

Pharmacokinetics

  • Absorption: Rapidly absorbed orally; Paracetamol peaks within 30–60 min, Cetirizine within ~1 hr, Caffeine within 30–60 min.
  • Distribution: Nimesulide ~99% protein bound; Paracetamol widely distributed with low protein binding; Cetirizine minimal CNS penetration; Caffeine crosses blood-brain barrier.
  • Metabolism: Nimesulide metabolized hepatically via CYP enzymes; Paracetamol via glucuronidation & sulfation; Cetirizine limited hepatic metabolism; Phenylpropanolamine minimal metabolism; Caffeine via CYP1A2.
  • Excretion: Primarily renal excretion for Paracetamol, Cetirizine, Phenylpropanolamine, and Caffeine; Nimesulide via urine and bile.
  • Half-life: Nimesulide ~2–5 hrs; Paracetamol ~2–3 hrs; Cetirizine ~8–10 hrs; Phenylpropanolamine ~3–4 hrs; Caffeine ~3–6 hrs.

Side Effects

  • Gastrointestinal upset (Nimesulide/Paracetamol)
  • Hepatotoxicity risk with prolonged use (Nimesulide/Paracetamol)
  • Drowsiness or sedation (Cetirizine)
  • Palpitations, hypertension (Phenylpropanolamine)
  • Insomnia, nervousness (Caffeine)
  • Rare allergic reactions

Contraindication

  • Hepatic impairment (Nimesulide/Paracetamol)
  • Hypersensitivity to NSAIDs or antihistamines
  • Hypertension and cardiovascular disease (Phenylpropanolamine)
  • Pregnancy & lactation (use cautiously)

Interaction

  • Alcohol increases hepatotoxicity risk with Paracetamol/Nimesulide
  • Phenylpropanolamine may interact with MAO inhibitors
  • Caffeine may enhance stimulant effects of other drugs
  • Cetirizine may cause additive sedation with CNS depressants

Warning and Precautions

  • Do not exceed recommended daily dose
  • Monitor liver function during prolonged use
  • Use cautiously in patients with cardiovascular disorders
  • Do not combine with other CNS stimulants

Dosage
