Mechanism

Naltrexone is a pure opioid antagonist that competitively binds to mu, kappa, and delta opioid receptors. By blocking these receptors, it prevents opioids from exerting their euphoric and reinforcing effects. In alcohol dependence, it modulates dopaminergic pathways in the mesolimbic system, reducing cravings and the rewarding effects of alcohol consumption.

Pharmacokinetics

  • Absorption: Rapidly absorbed after oral administration; peak plasma concentration within 1 hour.
  • Distribution: Widely distributed; plasma protein binding approximately 21%. Crosses the blood-brain barrier.
  • Metabolism: Extensively metabolized in the liver to active metabolite 6-beta-naltrexol.
  • Elimination: Excreted primarily via urine (renal clearance of metabolites).
  • Half-life: Naltrexone ~4 hours; active metabolite 6-beta-naltrexol ~13 hours, contributing to prolonged activity.
