Mechanism

Metamizole is a pyrazolone derivative that acts as a prodrug. Its active metabolites inhibit cyclooxygenase (COX) and prostaglandin synthesis, producing analgesic and antipyretic effects. It also has spasmolytic properties, making it effective in colic pain.

Pharmacokinetics

  • Absorption: Rapidly absorbed after oral administration.
  • Distribution: Widely distributed; crosses blood-brain barrier.
  • Metabolism: Converted to active metabolites in the liver.
  • Elimination: Excreted via urine.
  • Half-life: Approximately 2–3 hours.

Side Effects

  • Agranulocytosis (rare but serious)
  • Hypotension (especially with IV use)
  • Allergic reactions
  • Gastrointestinal discomfort

Contraindication

  • Hypersensitivity to pyrazolones
  • History of agranulocytosis
  • Severe hepatic or renal impairment

Interaction

  • CYP inducers/inhibitors: May alter metabolism.
  • Alcohol: Increased risk of GI irritation.
  • Other NSAIDs: Additive risk of adverse effects.
  • Anticoagulants: Potential increased bleeding risk.

Warning and Precautions

  • Hypersensitivity to metamizole, pyrazolones, or pyrazolidines.
  • Pregnancy and lactation, Severe hepatic or renal impairment.
  • Monitor blood counts regularly due to risk of agranulocytosis.
  • Use only under medical supervision.
  • Not recommended for long-term use.
  • Discontinue immediately if signs of infection or blood dyscrasia occur.
