Losartan + Hydrochlorothiazide

Tablet

This fixed-dose combination provides dual antihypertensive action. Losartan, an ARB, reduces vasoconstriction and aldosterone secretion, while Hydrochlorothiazide, a thiazide diuretic, lowers blood pressure by promoting sodium and water excretion. Together, they effectively control hypertension and reduce cardiovascular risk.

Mechanism

Losartan:Blocks angiotensin II type 1 (AT1) receptors, reducing vasoconstriction and aldosterone secretion. Hydrochlorothiazide:Inhibits sodium reabsorption in the distal tubules, increasing sodium and water excretion, thereby lowering blood pressure.

Pharmacokinetics

  • Absorption: Both drugs are well absorbed orally.
  • Distribution: Losartan highly protein bound; Hydrochlorothiazide moderately bound.
  • Metabolism: Losartan metabolized hepatically to active metabolite; Hydrochlorothiazide minimally metabolized.
  • Elimination: Losartan via urine/feces; Hydrochlorothiazide via urine.
  • Half-life: Losartan ≈ 2 hours (active metabolite 6–9 hours); Hydrochlorothiazide ≈ 6–15 hours.

Side Effects

  • Dizziness
  • Fatigue
  • Hyperkalemia (Losartan)
  • Hypokalemia (Hydrochlorothiazide)
  • Headache
  • Rare: angioedema, renal impairment

Contraindication

  • Hypersensitivity to Losartan, Hydrochlorothiazide, or excipients
  • Pregnancy (especially 2nd and 3rd trimester)
  • Severe hepatic or renal impairment
  • Anuria

Interaction

  • Potassium-sparing diuretics / supplements: Increased risk of hyperkalemia with Losartan.
  • NSAIDs: May reduce antihypertensive effect and impair renal function.
  • Other antihypertensives: Additive hypotensive effect.
  • Digitalis glycosides: Risk of arrhythmias with Hydrochlorothiazide-induced hypokalemia.

Warning and Precautions

  • Monitor blood pressure, renal function, and electrolytes regularly.
  • Check serum potassium and sodium levels during therapy.
  • Discontinue immediately if pregnancy is detected.
  • Use cautiously in patients with renal artery stenosis.
