Losartan + Amlodipine

Tablet

This fixed-dose combination provides dual antihypertensive action. Losartan, an ARB, reduces vasoconstriction and aldosterone secretion, while Amlodipine, a calcium channel blocker, relaxes vascular smooth muscle. Together, they effectively lower blood pressure and reduce cardiovascular risk.

Mechanism

Losartan: Blocks angiotensin II type 1 (AT1) receptors, reducing vasoconstriction and aldosterone secretion. Amlodipine:Inhibits calcium influx into vascular smooth muscle, causing vasodilation and lowering blood pressure.

Pharmacokinetics

  • Absorption: Losartan rapidly absorbed; Amlodipine has high oral bioavailability.
  • Distribution: Both are highly protein bound.
  • Metabolism: Losartan via CYP2C9/CYP3A4; Amlodipine via CYP3A4.
  • Elimination: Losartan via urine/feces; Amlodipine via urine.
  • Half-life: Losartan ≈ 2 hours (active metabolite 6–9 hours); Amlodipine ≈ 30–50 hours.

Side Effects

  • Dizziness
  • Fatigue
  • Peripheral edema
  • Headache
  • Hyperkalemia (due to Losartan)
  • Rare: angioedema, hypotension

Contraindication

  • Hypersensitivity to Losartan, Amlodipine, or excipients
  • Pregnancy (especially 2nd and 3rd trimester)
  • Severe hepatic impairment
  • Cardiogenic shock (Amlodipine contraindicated)

Interaction

  • Potassium-sparing diuretics / supplements: Increased risk of hyperkalemia with Losartan.
  • NSAIDs: May reduce antihypertensive effect and impair renal function.
  • Other antihypertensives: Additive hypotensive effect.
  • CYP3A4 inhibitors: Increase plasma levels of Amlodipine.

Warning and Precautions

  • Monitor blood pressure and renal function regularly.
  • Check serum potassium levels during therapy.
  • Discontinue immediately if pregnancy is detected.
  • Use cautiously in patients with hepatic impairment.
  • Monitor for peripheral edema due to Amlodipine.
