Mechanism

Lisinopril inhibits angiotensin‑converting enzyme (ACE), reducing angiotensin II levels and lowering blood pressure. Hydrochlorothiazide increases urinary excretion of sodium and water, reducing plasma volume. Together, they provide additive antihypertensive effects.

Pharmacokinetics

  • Lisinopril Absorption: Oral bioavailability ~25%; peak plasma concentration in 6–8 hours.
  • Lisinopril Distribution: Not protein bound; widely distributed.
  • Lisinopril Excretion: Excreted unchanged via kidneys; half‑life ~12 hours.
  • Hydrochlorothiazide Absorption: Well absorbed orally; peak plasma concentration in 1.5–4 hours.
  • Hydrochlorothiazide Bioavailability: ~60–80%.
  • Hydrochlorothiazide Excretion: Primarily excreted unchanged in urine; half‑life ~6–15 hours.

Side Effects

  • Dizziness
  • Fatigue
  • Hypotension
  • Hypokalemia (from Hydrochlorothiazide)
  • Hyperkalemia (from Lisinopril)
  • Headache

Contraindication

  • Hypersensitivity to Lisinopril, Hydrochlorothiazide, or excipients
  • Pregnancy (teratogenic risk with ACE inhibitors)
  • Anuria or severe renal impairment
  • History of angioedema with ACE inhibitors

Interaction

  • Potassium‑sparing diuretics or supplements (risk of hyperkalemia)
  • NSAIDs (may reduce antihypertensive effect)
  • Other antihypertensives (additive effect)
  • Digitalis glycosides (risk of arrhythmias with electrolyte imbalance)

Warning and Precautions

  • Monitor renal function and electrolytes regularly
  • Use under medical supervision only
  • Caution in patients with diabetes or gout
