Levodopa + Carbidopa

Tablet

Levodopa + Carbidopa is the gold-standard therapy for Parkinson’s disease. Levodopa replenishes dopamine in the brain, while Carbidopa prevents peripheral breakdown of Levodopa, enhancing its efficacy and reducing side effects such as nausea and hypotension.

Mechanism

Levodopa is a precursor of dopamine that crosses the blood-brain barrier and is converted to dopamine in the CNS, replenishing depleted levels in Parkinson’s disease. Carbidopa is a peripheral dopa-decarboxylase inhibitor that prevents premature conversion of Levodopa to dopamine outside the brain, increasing CNS availability and reducing peripheral side effects.

Pharmacokinetics

  • Absorption: Rapid from small intestine; affected by dietary protein
  • Distribution: Crosses blood-brain barrier as Levodopa
  • Metabolism: Levodopa metabolized by dopa-decarboxylase; Carbidopa inhibits peripheral metabolism
  • Half-life: Levodopa ~1.5 hours; extended with Carbidopa
  • Excretion: Metabolites excreted in urine

Side Effects

  • Nausea and vomiting
  • Orthostatic hypotension
  • Dyskinesias (involuntary movements)
  • Psychiatric disturbances (hallucinations, confusion)
  • Cardiac arrhythmias (rare)

Contraindication

  • Narrow-angle glaucoma
  • History of melanoma or undiagnosed skin lesions
  • Hypersensitivity to Levodopa or Carbidopa

Interaction

  • MAO inhibitors: risk of hypertensive crisis
  • Antipsychotics: may reduce efficacy
  • Iron salts: reduce absorption
  • Antihypertensives: additive hypotensive effect

Warning and Precautions

  • Monitor for dyskinesias and psychiatric symptoms
  • Use caution in patients with cardiovascular disease
  • Adjust dose gradually to minimize side effects
  • Protein-rich meals may interfere with absorption

Dosage

As directed by the Physician.
