Mechanism

Indapamide inhibits sodium reabsorption in the distal tubules, leading to increased excretion of sodium, chloride, and water. This reduces plasma volume and lowers blood pressure.

Pharmacokinetics

  • Absorption: Rapidly absorbed after oral administration.
  • Bioavailability: ~93%.
  • Metabolism: Extensively metabolized in the liver.
  • Excretion: Mainly via urine and feces.
  • Half-life: ~14–18 hours.

Side Effects

  • Dizziness
  • Headache
  • Muscle cramps
  • Electrolyte imbalance (low potassium/sodium)

Contraindication

  • Severe renal impairment
  • Severe hepatic impairment
  • Anuria
  • Hypersensitivity to indapamide or sulfonamides

Interaction

  • Other antihypertensives (additive effect)
  • NSAIDs (may reduce diuretic efficacy)
  • Digitalis glycosides (risk of arrhythmia with hypokalemia)
  • Lithium (risk of toxicity)

Warning and Precautions

  • Monitor electrolytes regularly
  • Use cautiously in elderly patients
  • Assess renal and hepatic function before initiation
  • Avoid alcohol (risk of hypotension)

Dosage

As directed by the Physician.
