Mechanism

Imipramine inhibits the reuptake of serotonin and norepinephrine at presynaptic terminals, increasing their availability in the synaptic cleft. This enhances neurotransmission and improves mood. It also has anticholinergic and antihistaminic properties, contributing to its sedative and side-effect profile.

Pharmacokinetics

  • Absorption: Well absorbed orally
  • Distribution: Highly protein bound (~95%)
  • Metabolism: Extensively metabolized in the liver to active metabolite desipramine
  • Half-life: ~8–20 hours
  • Excretion: Metabolites excreted in urine

  • Drowsiness
  • Dry mouth
  • Constipation
  • Weight gain
  • Orthostatic hypotension

  • Recent myocardial infarction
  • Cardiac conduction abnormalities
  • Hypersensitivity to Imipramine or TCAs
