Mechanism

Glipizide binds to sulfonylurea receptors on pancreatic beta cells, closing ATP-sensitive potassium channels and triggering insulin release. Metformin decreases hepatic gluconeogenesis, enhances peripheral glucose uptake, and improves insulin sensitivity.

Pharmacokinetics

  • Glipizide: Rapid absorption, peak plasma concentration in 1–3 hours, metabolized in liver, half-life ~2–4 hours.
  • Metformin: Absorbed in small intestine, bioavailability ~50–60%, excreted unchanged in urine, half-life ~6 hours.

Side Effects

  • Hypoglycemia (due to Glipizide)
  • Gastrointestinal upset (due to Metformin)
  • Lactic acidosis (rare, serious)
  • Weight gain (possible)

Contraindication

  • Hypoglycemia (due to Glipizide)
  • Gastrointestinal upset (due to Metformin)
  • Lactic acidosis (rare, serious)
  • Weight gain (possible)

Interaction

  • Alcohol (increased risk of hypoglycemia and lactic acidosis)
  • Other antidiabetic drugs (additive effect)
  • CYP2C9 inhibitors (may increase Glipizide levels)
  • Nephrotoxic drugs (increase risk with Metformin)

Warning and Precautions

  • Use under medical supervision only
  • Monitor blood glucose regularly
  • Assess renal function before starting Metformin
  • Caution in elderly patients prone to hypoglycemia
