Mechanism

Glimepiride stimulates pancreatic beta cells to release insulin, while Pioglitazone improves insulin sensitivity in peripheral tissues by activating PPAR-γ receptors.

Pharmacokinetics

  • Absorption: Well absorbed orally.
  • Metabolism: Glimepiride metabolized in liver (CYP2C9); Pioglitazone extensively metabolized hepatically.
  • Excretion: Primarily via urine and feces.
  • Half-life: Glimepiride ~5–8 hours; Pioglitazone ~16–24 hours.

Side Effects

  • Hypoglycemia
  • Weight gain
  • Edema
  • Headache
  • Rare: hepatotoxicity, heart failure risk

Contraindication

  • Hypersensitivity to sulfonylureas or thiazolidinediones
  • Type 1 Diabetes Mellitus
  • Severe hepatic impairment
  • Heart failure (NYHA Class III/IV)

Interaction

  • Other antidiabetic drugs (risk of hypoglycemia)
  • CYP2C9 inhibitors/inducers (altered Glimepiride metabolism)
  • Insulin (increased risk of edema)

Warning and Precautions

    • Monitor blood glucose regularly
    • Assess liver function before and during therapy
    • Use with caution in patients with cardiovascular disease
    • Not recommended during pregnancy unless essential
