Mechanism

Gliclazide binds to sulfonylurea receptors on pancreatic beta cells, closing ATP-sensitive potassium channels. This depolarizes the cell membrane, opening calcium channels, and triggering insulin release.

Pharmacokinetics

  • Absorption: Rapidly absorbed; peak plasma concentration in 4–6 hours.
  • Bioavailability: ~60–100%.
  • Distribution: Highly protein bound (~85–97%).
  • Metabolism: Extensively metabolized in the liver.
  • Excretion: Metabolites excreted via urine.
  • Half-life: ~10–12 hours (modified release formulations allow once-daily dosing).

Side Effects

  • Hypoglycemia
  • Weight gain
  • Gastrointestinal upset
  • Skin rash (rare)

Contraindication

  • Type 1 Diabetes Mellitus
  • Diabetic ketoacidosis
  • Severe renal or hepatic impairment
  • Hypersensitivity to sulfonylureas

Interaction

  • Alcohol (increased risk of hypoglycemia)
  • Other antidiabetic drugs (additive effect)
  • CYP2C9 inhibitors (may increase gliclazide levels)

Warning and Precautions

  • Use under medical supervision only
  • Monitor blood glucose regularly
  • Caution in elderly patients prone to hypoglycemia

Dosage

As directed by the Physician.
