Gliclazide + Metformin

Tablet

This fixed-dose combination provides dual benefits in type 2 diabetes management. Gliclazide, a sulfonylurea, stimulates insulin secretion from pancreatic beta cells, while Metformin, a biguanide, reduces hepatic glucose production and improves insulin sensitivity.

Mechanism

Gliclazide binds to sulfonylurea receptors on pancreatic beta cells, closing ATP-sensitive potassium channels and triggering insulin release. Metformin decreases hepatic gluconeogenesis, enhances peripheral glucose uptake, and improves insulin sensitivity.

Pharmacokinetics

  • Gliclazide: Rapidly absorbed; peak plasma concentration in 4–6 hours; highly protein bound (~85–97%); metabolized in liver; excreted via urine; half-life ~10–12 hours.
  • Metformin: Absorbed mainly in small intestine; bioavailability ~50–60%; not metabolized; excreted unchanged in urine; half-life ~6 hours.

Side Effects

  • Hypoglycemia (due to Gliclazide)
  • Weight gain (possible)
  • Gastrointestinal upset (due to Metformin)
  • Lactic acidosis (rare, serious)

Contraindication

  • Type 1 Diabetes Mellitus
  • Diabetic ketoacidosis
  • Severe renal or hepatic impairment
  • Hypersensitivity to sulfonylureas or biguanides

Interaction

  • Alcohol (increased risk of hypoglycemia and lactic acidosis)
  • Other antidiabetic drugs (additive effect)
  • CYP2C9 inhibitors (may increase gliclazide levels)
  • Nephrotoxic drugs (increase risk with Metformin)

Warning and Precautions

  • Use under medical supervision only
  • Monitor blood glucose regularly
  • Assess renal function before starting Metformin
  • Caution in elderly patients prone to hypoglycemia

Dosage

As directed by the Physician.
