Mechanism

Glibenclamide binds to sulfonylurea receptors on pancreatic beta cells, closing ATP-sensitive potassium channels. This depolarizes the cell membrane, opens calcium channels, and triggers insulin release.

Pharmacokinetics

  • Absorption: Rapidly absorbed after oral administration.
  • Peak Plasma Concentration: 2–4 hours.
  • Protein Binding: Highly bound (~99%) to plasma proteins.
  • Metabolism: Extensively metabolized in the liver.
  • Excretion: Metabolites excreted via urine and bile.
  • Half-life: ~10 hours (longer in elderly or renal impairment).

Side Effects

  • Hypoglycemia (most significant risk)
  • Weight gain
  • Gastrointestinal upset (nausea, vomiting)
  • Rare: Allergic skin reactions, liver enzyme changes

Contraindication

  • Type 1 Diabetes Mellitus
  • Diabetic ketoacidosis
  • Severe renal or hepatic impairment
  • Hypersensitivity to sulfonylureas

Interaction

  • Alcohol (increases risk of hypoglycemia)
  • Other antidiabetic drugs (additive effect)
  • Beta-blockers (may mask hypoglycemia symptoms)
  • CYP2C9 inhibitors (increase drug levels)

Warning and Precautions

    • Use under medical supervision only
    • Monitor blood glucose regularly
    • Adjust dose in elderly or renal/hepatic impairment
    • Avoid skipping meals to reduce hypoglycemia risk
