Mechanism

Faropenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death. It is stable against many beta-lactamases, making it effective against resistant organisms.

Pharmacokinetics

  • Absorption: Rapid oral absorption; peak plasma concentration within 1 hour.
  • Distribution: Widely distributed in tissues; moderate protein binding.
  • Metabolism: Minimal hepatic metabolism.
  • Elimination: Primarily renal excretion.
  • Half-life: Approximately 1 hour.

Side Effects

  • Nausea
  • Diarrhea
  • Abdominal pain
  • Skin rash
  • Rare: hypersensitivity reactions

Contraindication

  • Hypersensitivity to penems, carbapenems, or other beta-lactam antibiotics
  • Severe renal impairment without dose adjustment
  • History of severe allergic reactions to beta-lactams

Interaction

  • Probenecid: Increases plasma concentration of Faropenem.
  • Other beta-lactams: Possible additive effect.
  • Nephrotoxic drugs: Increased risk of renal adverse effects.

Warning and Precautions

  • Monitor renal function during therapy.
  • Use cautiously in patients with history of beta-lactam allergy.
  • Discontinue if severe diarrhea or hypersensitivity occurs.
