Mechanism

Etodolac selectively inhibits cyclooxygenase-2 (COX-2), reducing prostaglandin synthesis and thereby decreasing inflammation, pain, and swelling. Tolperisone stabilizes neuronal membranes and inhibits abnormal reflex activity in the spinal cord, reducing muscle spasm without causing sedation.

Pharmacokinetics

  • Etodolac: Rapidly absorbed; peak plasma concentration in 1–2 hours; highly protein bound; metabolized in liver; excreted via urine and feces; half-life ~7 hours.
  • Tolperisone: Absorbed orally; peak plasma concentration in 1.5 hours; metabolized in liver and kidney; excreted via urine; half-life ~2–2.5 hours.

Side Effects

  • Gastrointestinal discomfort
  • Nausea
  • Dizziness
  • Skin rash (rare)
  • Muscle weakness (rare with Tolperisone)

Contraindication

  • Hypersensitivity to NSAIDs or Tolperisone
  • Severe hepatic or renal impairment
  • History of gastrointestinal bleeding or peptic ulcer
  • Pregnancy and lactation (use with caution)

Interaction

  • Alcohol (increased risk of gastric bleeding and CNS effects)
  • Anticoagulants (enhanced bleeding risk)
  • Other NSAIDs (additive GI toxicity)
  • Muscle relaxants (additive effect with Tolperisone)

Warning and Precautions

  • Use under medical supervision only
  • Monitor liver and kidney function during therapy
  • Use lowest effective dose for shortest duration
