Eltrombopag

Tablet

Eltrombopag is an oral thrombopoietin receptor agonist. It stimulates platelet production by activating the thrombopoietin receptor (c-Mpl), used in chronic ITP, aplastic anemia, and hepatitis C–related thrombocytopenia.

Mechanism

Eltrombopag binds to the transmembrane domain of the thrombopoietin receptor (c-Mpl) on megakaryocyte precursors. This stimulates proliferation and differentiation of megakaryocytes, increasing platelet production.

Pharmacokinetics

  • Absorption: Oral bioavailability ~52%; reduced with high-calcium meals.
  • Distribution: Highly protein bound (>99%).
  • Metabolism: Hepatic metabolism via CYP1A2 and UGT1A1.
  • Elimination: Excreted in feces (~59%) and urine (~31%).
  • Half-life: ~21–32 hours.

Side Effects

  • Headache
  • Nausea
  • Fatigue
  • Diarrhea
  • Hepatotoxicity (elevated liver enzymes)
  • Thromboembolic events (rare)
  • Contraindication

    • Hypersensitivity to Eltrombopag
    • Severe hepatic impairment (unless benefits outweigh risks)
    • Pregnancy and lactation — use cautiously

    Interaction

    • Polyvalent cations (Ca, Mg, Fe): Reduce absorption; avoid taking with dairy or supplements.
    • CYP substrates: Potential interactions with drugs metabolized by CYP1A2.
    • Hepatotoxic drugs: Additive risk of liver injury.

    Warning and Precautions

    • Monitor liver function tests regularly.
    • Assess platelet counts frequently to avoid thrombosis.
    • Do not use for myelodysplastic syndromes (risk of progression to AML).
    • Take on an empty stomach, away from dairy or mineral supplements.
