Doxofylline

Tablet

Doxofylline is a xanthine derivative with bronchodilator properties. It improves airflow by relaxing bronchial smooth muscles, reducing airway resistance, and enhancing oxygenation. Compared to theophylline, Doxofylline has fewer cardiovascular and gastrointestinal side effects, making it a safer option in long-term respiratory therapy.

Mechanism

Doxofylline inhibits phosphodiesterase enzymes, increasing cyclic AMP levels in bronchial smooth muscle. This leads to relaxation of airways, improved airflow, and reduced bronchospasm. Unlike theophylline, it has minimal cardiac stimulation and reduced gastrointestinal irritation.

Pharmacokinetics

  • Absorption: Rapid oral absorption
  • Distribution: Widely distributed in lung tissues
  • Metabolism: Hepatic metabolism
  • Elimination: Renal excretion
  • Half-life: ≈ 6–8 hours

Side Effects

  • Nausea
  • Headache
  • Insomnia
  • Rare: palpitations

Contraindication

  • Hypersensitivity to xanthine derivatives
  • Severe hepatic impairment

Interaction

  • Beta-agonists — additive bronchodilation
  • Corticosteroids — synergistic effect in asthma/COPD
  • Alcohol — may enhance CNS stimulation
  • Other xanthines — increased risk of side effects

Warning and Precautions

  • Monitor liver function during therapy
  • Use cautiously in patients with cardiovascular disease
  • Avoid excessive caffeine intake
