Doxofylline + Montelukast

Tablet

This fixed-dose combination provides dual benefits in respiratory care. Doxofylline, a xanthine derivative, acts as a bronchodilator with fewer side effects compared to theophylline. Montelukast, a leukotriene receptor antagonist, reduces airway inflammation and prevents bronchoconstriction. Together, they improve airflow, reduce symptoms, and enhance quality of life in asthma and COPD patients.

Mechanism

Doxofylline inhibits phosphodiesterase enzymes, increasing cyclic AMP levels in bronchial smooth muscle, leading to airway relaxation. Montelukast blocks leukotriene D4 receptors, reducing inflammation, mucus secretion, and bronchoconstriction. The combination provides synergistic control of airway obstruction and inflammation.

Pharmacokinetics

  • Doxofylline Absorption: Rapid oral absorption
  • Doxofylline Half-life: ≈ 6–8 hours
  • Montelukast Absorption: Rapid oral absorption, bioavailability ≈ 64%
  • Montelukast Half-life: ≈ 2.7–5.5 hours
  • Elimination: Both drugs undergo hepatic metabolism and renal/fecal excretion

Side Effects

  • Nausea
  • Headache
  • Insomnia
  • Abdominal discomfort
  • Rare: palpitations, mood changes

Contraindication

  • Hypersensitivity to xanthine derivatives or Montelukast
  • Severe hepatic impairment

Interaction

  • Doxofylline — interacts with beta-agonists, corticosteroids, alcohol, and other xanthines
  • Montelukast — interacts with CYP3A4 inducers (rifampicin, phenytoin) reducing efficacy
  • Use cautiously with hepatotoxic drugs

Warning and Precautions

  • Monitor liver function during therapy
  • Use cautiously in patients with cardiovascular disease
  • Not recommended for acute asthma attacks (maintenance therapy only)
