Mechanism

Donepezil selectively and reversibly inhibits acetylcholinesterase, the enzyme responsible for hydrolyzing acetylcholine. By increasing acetylcholine concentration in the synaptic cleft, it enhances cholinergic transmission in the cerebral cortex and hippocampus, improving cognition, memory, and daily functioning in Alzheimer’s patients.

Pharmacokinetics

  • Bioavailability: ~100%
  • Peak plasma concentration: 3–4 hours
  • Protein binding: ~95%
  • Metabolism: Hepatic via CYP2D6 and CYP3A4
  • Half-life: ~70 hours (allows once-daily dosing)
  • Excretion: Renal and biliary routes

Side Effects

  • Nausea
  • Diarrhea
  • Insomnia
  • Muscle cramps
  • Fatigue
  • Bradycardia (rare)

Contraindication

  • Hypersensitivity to Donepezil or piperidine derivatives
  • Severe hepatic impairment
  • Pregnancy and lactation (unless benefits outweigh risks)

Interaction

  • Anticholinergic drugs: Reduced efficacy
  • Cholinergic agonists: Additive effects
  • CYP3A4/CYP2D6 inhibitors (e.g., ketoconazole, quinidine): ↑ Donepezil levels
  • CYP inducers (e.g., phenytoin, carbamazepine): ↓ Donepezil levels

Warning and Precautions

  • Monitor cardiac function in patients with conduction abnormalities
  • Use caution in patients with asthma or COPD (cholinergic effects)
  • Risk of gastrointestinal bleeding in patients with ulcers
  • Adjust dose in hepatic impairment

Dosage

As directed by the Physician.
