Mechanism

Diphenhydramine: Antihistamine that reduces allergy symptoms and provides sedation.
Phenylephrine: Decongestant that constricts nasal blood vessels, relieving congestion.
Caffeine: CNS stimulant that reduces fatigue and enhances analgesic effect.
Paracetamol: Analgesic and antipyretic that reduces pain and fever.

Pharmacokinetics

  • Diphenhydramine: Rapidly absorbed; peak plasma concentration in 2–3 hours; metabolized in liver (CYP2D6); half-life ~4–8 hours; excreted in urine.
  • Phenylephrine: Low oral bioavailability (~40%); peak effect within 1–2 hours; metabolized extensively in gut and liver; half-life ~2–3 hours.
  • Caffeine: Almost completely absorbed; peak plasma concentration in 30–60 minutes; metabolized in liver (CYP1A2); half-life ~3–7 hours; excreted in urine.
  • Paracetamol: Rapid absorption; peak plasma concentration in 30–60 minutes; metabolized in liver (glucuronidation, sulfation); half-life ~2–3 hours; excreted in urine.

Side Effects

  • Drowsiness (due to Diphenhydramine)
  • Insomnia or jitteriness (due to Caffeine)
