Mechanism

Dienogest acts as a progestin that suppresses estrogen levels and inhibits the growth of endometrial tissue. It reduces inflammation and prevents the progression of endometriosis.

Pharmacokinetics

  • Absorption: Dienogest is rapidly and almost completely absorbed after oral administration. Peak plasma concentrations are reached within 1.5 hours.
  • Distribution: Dienogest is moderately bound to plasma proteins (about 90%) and does not bind specifically to sex hormone-binding globulin (SHBG). It is widely distributed in body tissues.
  • Metabolism: Dienogest is extensively metabolized in the liver, mainly via hydroxylation and conjugation pathways, producing inactive metabolites.
  • Elimination: Dienogest is excreted primarily in the form of metabolites via urine and feces. The elimination half-life is approximately 9–10 hours.
  • Special Populations:
    • Hepatic impairment: Use is contraindicated in severe liver disease.
    • Renal impairment: No significant effect on pharmacokinetics.
