Mechanism

Diclofenac inhibits cyclooxygenase (COX) enzymes, reducing prostaglandin synthesis and thereby decreasing inflammation, pain, and swelling. Paracetamol acts centrally on the hypothalamus to reduce fever and modulates pain pathways to enhance analgesic effect.

Pharmacokinetics

  • Diclofenac: Rapidly absorbed; peak plasma concentration in 1–2 hours; metabolized in liver; excreted via urine and bile; half-life ~2 hours.
  • Paracetamol: Rapidly absorbed; peak plasma concentration in 30–60 minutes; metabolized in liver; excreted via urine; half-life ~2–3 hours.

Side Effects

  • Gastrointestinal discomfort
  • Nausea
  • Dizziness
  • Skin rash (rare)

Contraindication

  • Hypersensitivity to NSAIDs or Paracetamol
  • Severe hepatic or renal impairment
  • History of gastric ulcers or bleeding

Interaction

  • Alcohol (increases risk of gastric bleeding and liver damage)
  • Anticoagulants (enhanced bleeding risk)
  • Other NSAIDs (avoid duplication)
  • Hepatotoxic drugs (increased risk of liver injury with Paracetamol)

Warning and Precautions

  • Use under medical supervision only
  • Monitor liver function in long-term use
  • Caution in patients with bleeding disorders

Dosage

As directed by the Physician.
