Mechanism

Dapagliflozin: Inhibits sodium-glucose co-transporter 2 (SGLT2) in proximal renal tubules → increases urinary glucose excretion → lowers plasma glucose.
Metformin: Decreases hepatic gluconeogenesis, increases peripheral glucose uptake, and reduces intestinal glucose absorption.

Pharmacokinetics

  • Dapagliflozin Absorption: Oral bioavailability ~78%; peak plasma levels in 2 hrs.
  • Metformin Absorption: Peak plasma levels in 2–3 hrs; bioavailability ~50–60%.
  • Distribution: Dapagliflozin ~91% protein bound; Metformin not protein bound.
  • Metabolism: Dapagliflozin metabolized hepatically (UGT1A9); Metformin not metabolized.
  • Excretion: Dapagliflozin via urine and feces; Metformin via renal excretion unchanged.
  • Half-life: Dapagliflozin ~12 hrs; Metformin ~6 hrs.

Side Effects

  • Genital mycotic infections (dapagliflozin)
  • Urinary tract infections
  • Hypoglycemia (especially with other antidiabetics)
  • Gastrointestinal upset (metformin)
  • Rare: Lactic acidosis (metformin)

Contraindication

  • Type 1 diabetes mellitus
  • Diabetic ketoacidosis
  • Severe renal impairment (eGFR < 30 mL/min/1.73 m²)
  • Hypersensitivity to either component

Interaction

  • Alcohol ↑ risk of lactic acidosis (metformin)
  • Diuretics ↑ risk of dehydration and hypotension (dapagliflozin)
  • Other antidiabetics ↑ risk of hypoglycemia

Warning and Precautions

  • Monitor renal function regularly
  • Advise patients on genital hygiene to reduce infection risk
  • Take with meals to reduce GI upset

Dosage
