Clomiphene

Tablet

Clomiphene is a selective estrogen receptor modulator (SERM) used primarily to induce ovulation in women with infertility due to anovulation or PCOS. It is also used off-label in male infertility to stimulate endogenous testosterone production.

Mechanism

Clomiphene binds to estrogen receptors in the hypothalamus, blocking negative feedback of estrogen. This increases GnRH secretion, stimulating pituitary release of FSH and LH, which promote follicular growth and ovulation. In men, it enhances LH and FSH secretion, stimulating testosterone and spermatogenesis.

Pharmacokinetics

  • Absorption: Well absorbed orally; peak plasma concentration in 5–7 hours
  • Distribution: Highly protein bound; accumulates in adipose tissue
  • Metabolism: Hepatic via CYP450 enzymes
  • Half-life: ~5–7 days
  • Excretion: Mainly via feces

    • Ovarian enlargement
    • Visual disturbances
    • Nausea and vomiting
    • Breast tenderness
    • Multiple pregnancies (risk)

      • Contraindication

      • Pregnancy
      • Liver disease
      • Uncontrolled thyroid or adrenal disorders
      • Ovarian cysts (not due to PCOS)

      Interaction

    • Concurrent use with other fertility drugs: ↑ risk of ovarian hyperstimulation
    • Alcohol: may worsen side effects

    Warning and Precautions

    • Monitor ovarian response via ultrasound
    • Risk of multiple pregnancies
    • Use caution in patients with visual disturbances
    • Limit therapy to recommended cycles
