Cilnidipine + Metoprolol ER

Tablet

This fixed-dose combination provides dual antihypertensive action. Cilnidipine, a calcium channel blocker, reduces vascular resistance, while Metoprolol ER, a beta-blocker, lowers heart rate and myocardial oxygen demand. Together, they provide effective blood pressure control and cardiovascular protection.

Mechanism

Cilnidipine: Dual L/N-type calcium channel blocker → reduces vascular resistance and sympathetic nerve activity.
Metoprolol ER: Selective β1-blocker → decreases heart rate, cardiac output, and myocardial oxygen demand.

Pharmacokinetics

  • Cilnidipine Absorption: Oral bioavailability ~40%; peak plasma levels in 2 hrs.
  • Metoprolol ER Absorption: Extended release formulation ensures steady plasma levels.
  • Distribution: Both widely distributed; Metoprolol ~12% protein bound.
  • Metabolism: Cilnidipine metabolized hepatically; Metoprolol via CYP2D6.
  • Excretion: Both primarily renal excretion.
  • Half-life: Cilnidipine ~2–5 hrs; Metoprolol ER ~6–9 hrs.

Side Effects

  • Dizziness
  • Fatigue
  • Bradycardia (Metoprolol)
  • Peripheral edema (Cilnidipine)
  • Rare: Hypotension, AV block

Contraindication

  • Severe bradycardia or AV block
  • Cardiogenic shock
  • Hypersensitivity to either component

Interaction

  • Other antihypertensives may potentiate hypotension
  • NSAIDs may reduce antihypertensive effect
  • Beta-blockers with verapamil/diltiazem ↑ risk of bradycardia

Warning and Precautions

  • Monitor blood pressure and heart rate regularly
  • Use cautiously in patients with asthma or COPD (Metoprolol)
  • Gradual withdrawal recommended to avoid rebound hypertension
