Mechanism

Cilnidipine blocks L-type calcium channels in vascular smooth muscle, causing vasodilation and lowering blood pressure. It also inhibits N-type calcium channels in sympathetic nerve endings, reducing norepinephrine release and sympathetic overactivity.

Pharmacokinetics

  • Absorption: Rapid oral absorption.
  • Distribution: Highly lipophilic, widely distributed.
  • Metabolism: Extensively metabolized in liver.
  • Elimination: Excreted via urine and feces.
  • Half-life: ~2–4 hours.

Side Effects

  • Dizziness
  • Headache
  • Peripheral edema
  • Flushing
  • Rare: hypotension, palpitations

Contraindication

  • Hypersensitivity to Cilnidipine
  • Severe hypotension
  • Cardiogenic shock
  • Pregnancy and lactation — use cautiously

Interaction

  • Other antihypertensives: Additive hypotensive effect.
  • Beta-blockers: May increase risk of bradycardia.
  • NSAIDs: May reduce antihypertensive efficacy.
  • CYP3A4 inhibitors: May increase plasma levels of Cilnidipine.

Warning and Precautions

  • Monitor blood pressure regularly.
  • Use cautiously in patients with hepatic impairment.
  • Advise patients to avoid abrupt discontinuation.
  • Adjust dose in elderly patients.
