Cabergoline

Tablet

Cabergoline is a potent dopamine D2 receptor agonist. It suppresses prolactin secretion from the pituitary gland, making it effective in treating hyperprolactinemia and related reproductive disorders. It is also used as adjunct therapy in Parkinson’s disease.

Mechanism

Cabergoline acts as a dopamine D2 receptor agonist. It inhibits prolactin secretion from the anterior pituitary gland, normalizing reproductive function. In Parkinson’s disease, it enhances dopaminergic activity in the brain, improving motor symptoms.

Pharmacokinetics

  • Absorption: Well absorbed orally.
  • Distribution: Highly protein bound (~40%).
  • Metabolism: Hepatic metabolism via hydrolysis.
  • Elimination: Excreted in feces and urine.
  • Half-life: ~63–69 hours (long duration of action).

Side Effects

  • Nausea, vomiting
  • Dizziness
  • Fatigue
  • Orthostatic hypotension
  • Rare: cardiac valvulopathy (long-term use)

Contraindication

  • Hypersensitivity to Cabergoline or ergot derivatives
  • Uncontrolled hypertension
  • History of cardiac valvular disorders
  • Pregnancy and lactation — use cautiously

Interaction

  • Antihypertensives: Additive hypotensive effect.
  • Macrolide antibiotics: May increase Cabergoline levels.
  • Antipsychotics: May antagonize dopamine agonist effects.

Warning and Precautions

  • Monitor blood pressure regularly, especially at initiation.
  • Perform echocardiography before and during long-term therapy.
  • Use cautiously in patients with hepatic impairment.
  • Advise patients to avoid sudden posture changes to reduce dizziness.

Dosage
