Mechanism

Beta-Carotene:

  • Acts as a precursor to Vitamin A (retinol), supporting vision, epithelial integrity, and immune function.
  • Functions as a potent antioxidant, neutralizing free radicals and reducing oxidative stress.
  • Regulates gene expression related to cell differentiation and growth.

Alfacalcidol:

  • A synthetic analogue of Vitamin D3, rapidly converted in the liver to calcitriol (active form).
  • Enhances intestinal absorption of calcium and phosphate, promoting bone mineralization.
  • Regulates parathyroid hormone secretion, reducing bone resorption.
  • Supports neuromuscular function and immune modulation.

Calcium Carbonate:

  • Provides elemental calcium, essential for bone strength and density.
  • Neutralizes gastric acid, aiding in calcium absorption.
  • Supports muscle contraction, nerve transmission, and blood clotting.

Pharmacokinetics

  • Absorption: Beta-Carotene absorbed in intestine with dietary fat; Alfacalcidol absorbed rapidly; Calcium Carbonate requires acidic medium for optimal absorption.
  • Distribution: Beta-Carotene stored in liver and adipose tissue; Alfacalcidol distributed systemically; Calcium incorporated into bone and extracellular fluid.
  • Metabolism: Beta-Carotene converted to retinol; Alfacalcidol hydroxylated in liver to calcitriol; Calcium undergoes minimal metabolism.
  • Elimination: Beta-Carotene via bile; Alfacalcidol metabolites via urine; Calcium excreted via urine and feces.

Side Effects

  • Beta-Carotene: Carotenemia (orange skin discoloration), GI upset
  • Alfacalcidol: Hypercalcemia, headache, nausea
  • Calcium Carbonate: Constipation, bloating, kidney stones (long-term high dose)

Contraindication

  • Hypersensitivity to any component
  • Hypercalcemia or hypervitaminosis D
  • Severe renal impairment

Interaction

  • Beta-Carotene absorption reduced by alcohol and smoking
  • Alfacalcidol interacts with thiazide diuretics, digoxin
  • Calcium Carbonate reduces absorption of tetracyclines, bisphosphonates, levothyroxine

Warning and Precautions

  • Monitor serum calcium and vitamin D levels during therapy.
  • Use cautiously in patients with renal impairment or kidney stones.
  • Long-term use requires periodic bone density evaluation.
  • Pregnant and lactating women should use only under medical advice.
  • Store in a cool, dry place away from sunlight.
