Benidipine

Tablet

Benidipine is a long-acting calcium channel blocker (CCB) that inhibits L-, T-, and N-type calcium channels. It is effective in lowering blood pressure, improving coronary blood flow, and reducing cardiac workload. Benidipine is widely prescribed for hypertension and angina, offering dual cardiovascular benefits.

Mechanism

Benidipine blocks L-, T-, and N-type calcium channels in vascular smooth muscle and cardiac tissue. This reduces calcium influx, leading to vasodilation, decreased peripheral resistance, and improved coronary blood flow.

Pharmacokinetics

  • Absorption: Well absorbed orally
  • Distribution: Widely distributed in vascular tissues
  • Metabolism: Hepatic metabolism via CYP3A4
  • Elimination: Primarily fecal excretion
  • Half-life: 8–10 hours

Side Effects

  • Dizziness
  • Headache
  • Peripheral edema
  • Flushing
  • Rare gingival hyperplasia

Contraindication

  • Severe hypotension
  • Cardiogenic shock
  • Hypersensitivity to Benidipine

Interaction

  • CYP3A4 inhibitors (increase Benidipine levels)
  • CYP3A4 inducers (reduce Benidipine efficacy)
  • Other antihypertensives (additive blood pressure lowering)

Warning and Precautions

  • Monitor blood pressure regularly
  • Use cautiously in patients with hepatic impairment
  • Adjust dose in elderly patients
