Azithromycin

Tablet

Azithromycin is a broad-spectrum macrolide antibiotic used to treat a variety of bacterial infections. It works by inhibiting bacterial protein synthesis, thereby stopping the growth of bacteria. It is highly effective in respiratory infections, skin infections, and sexually transmitted diseases. Azithromycin offers the advantage of once-daily dosing and shorter treatment duration, improving patient compliance.

Mechanism

Azithromycin binds to the 50S ribosomal subunit of susceptible bacteria and inhibits RNA-dependent protein synthesis. This action prevents bacterial growth and replication, making it bacteriostatic in nature. At higher concentrations, it may exhibit bactericidal activity against certain organisms.

Pharmacokinetics

  • Absorption: Rapid oral absorption with good bioavailability
  • Distribution: Extensive tissue penetration and high intracellular concentration
  • Metabolism: Minimal hepatic metabolism
  • Elimination: Primarily excreted unchanged in bile; small amount via urine
  • Half-life: Long half-life (~68 hours)

Side Effects

  • Nausea
  • Diarrhea
  • Abdominal pain
  • Headache

Contraindication

    <
  • Hypersensitivity to macrolide antibiotics
  • Severe liver disease

Interaction

  • Antacids (reduce absorption)
  • Warfarin (increase bleeding risk)
  • Other QT-prolonging drugs

Warning and Precautions

  • Use cautiously in liver impairment
  • May prolong QT interval (cardiac risk)
  • Complete full antibiotic course
